Health, Housing, and Hope: The Casa Project’s Next Chapter

At MCH we know that housing represents health, stability, and care for individuals and families living in poverty. Housing instability can quickly lead to declining health, frequent emergency room visits, and additional challenges that create barriers to regain stability. We’re proud to share that through The Casa Project MCH is now an official provider under MassHealth’s newly developed Health-Related Social Needs (HRSN) framework. This innovative model designed to serve MassHealth members with the highest needs focused on those who are frequent users of emergency departments and live with chronic conditions or permanent diagnoses that put them at greater risk of worsening housing and nutritional insecurity. The HRSN framework builds on the foundation of the Flexible Services Program, which MCH proudly implemented from 2021 to 2024. Through that initiative, we assisted over 5,000 households across the state who were experiencing homelessness or housing instability. Now, as part of the newly launched HRSN initiative, we continue this work as the largest HRSN provider for housing services in partnership with 14 of the 17 Accountable Care Organizations (ACOs) statewide, providing housing support to 13 ACOs and both housing and nutrition services to 1 ACO.

 

Our work under The Casa Project is not only about connecting people with housing, but also about ensuring families and individuals achieve long-term stabilization. Over the past year, through contracts, collaborations, and partnerships at the local, regional, and state levels, the program assisted 2,287 households retain or obtain stable housing. Many avoided evictions entirely, while others were able to secure affordable housing or improved their financial circumstances to prevent homelessness or displacement. The outcomes of the program speak to the heart of our work, ensuring that those most at risk are not forgotten, but instead met with compassion and receive evidence-based support tailored to their unique needs. From homelessness prevention to nutrition access, The Casa Project is creating a stronger, healthier future and we remain committed to utilizing the HRSN framework as a new chapter for the program to expand reach, strengthen partnerships, and assist with care and purpose the communities of Massachusetts.
